A meeting of the Utility Service Board was held on Monday, November 7, 2016 at 5:00 p.m. Attendees: Louis Zimmer, John Widmann, Chris Miller, and Clerk Treasurer Cathy Mittendorf. Also present: Attorney David Hawk, Superintendent Don Papai, and Derek Frederickson, Engineering Resources. There were 8 visitors and 1 reporter.

Louis Zimmer called the meeting to order with the Pledge of Allegiance.

USB ACTIONS

Chris Miller made a motion to accept October 3, 2016 meeting minutes, John Widmann seconded. Louis Zimmer-abstain, John Widmann-yes, Chris Miller-yes.

Louis Zimmer made a motion to accept water and sewer claims for October 17, 2016 and November 7, 2016, Chris Miller seconded. All in favor.

Louis Zimmer made a motion to approve an additional $15,000 being held in retainage to Engineering Resources for the remainder of 2016, John Widmann seconded. All in favor.

Louis Zimmer made a motion to accept Fetters allowance authorization in the amount of $1753, John Widmann seconded. All in favor. This deletes 2 stop plates and frames to 1 surface mount non-self-contained aluminum slide gate with a manual hand wheel.

Louis Zimmer made a motion to accept pay application # 5 to Geiger Excavating, Inc. in the amount of $240,703.14, Chris Miller seconded. All in favor. This is phase 2B Effluent Force Main.

John Widmann made a motion to accept pay application # 3 to Geiger Excavating, Inc. in the amount of $10,269.00, Chris Miller seconded. All in favor. This is phase 2B Effluent Force Main.

Louis Zimmer made a motion to accept pay application # 6 to Fetters Construction, Inc. in the amount of $689,947.92, John Widmann seconded. All in favor. This is for the WWTP phase 2A.

Don Papai
   1. Towne Square Lift Station is has been installed with some adjustments needed.
   2. Working on a new 2017 salary benefit package.

Derek Frederickson
   1. The $15,000 being held for retainage to Engineering Resources was approved.
   2. The WWTP is roughly 26% complete with a comfortable contingency left in the contract
   3. Pay allowance # 6 for $1753 was voted and accepted.
   4. Pay application # 5 for $240,703.14 from Geiger Excavating was presented and approved.
   5. Pay application # 3 for $10,269 from Geiger Excavating was presented and approved.
   6. Pay application # 6 for $689,947.92 from Fetters Construction was presented and approved.
